Practice & Progress Record
A simple record of each practice session — what you practised, where, how much assistance you needed, what went well and the next step.
Progress is easiest to see when it is written down. Each time you practise a skill or routine, this record captures the date, the setting, the assistance or prompts used, what went well and the barrier or next step.
Recording assistance honestly matters: it is how you and your team see support reducing over time — or notice when it is not.
The reflection at the bottom asks what feels easier now, what still needs practice, and where else you would like to use the skill.
