12-Week Functional Review
The structured review that anchors the program: baseline against current performance, change in assistance, safety, and the decision for the next 12 weeks.
Every 12–13 weeks, the participant and the team review progress against each goal and the routines that hold daily life together. The review starts with the participant's voice.
For each goal — plus routine maintenance and community participation — it compares baseline with current performance, records the change in assistance and the consistency of safety, and sets the focus for the next 12 weeks.
It ends with a clear decision: continue, adapt the teaching or environment, generalise to another setting, maintain and consolidate, or seek additional professional review.
