My Daily Routine Planner
A practical tool for organising everyday routines, identifying useful reminders and prompts, and building more consistent participation in daily life.
The Daily Routine Planner is not a generic timetable or a lifestyle checklist. It is an IMHC practical learning and implementation tool designed to help a person turn learning, professional recommendations and their own established strategies into everyday routines.
A person may understand what they need to do but still experience difficulty remembering, initiating, sequencing or consistently completing a routine in everyday life. The planner makes those steps visible.
For each part of the day you record what you want or need to do, the reminder or cue that may help, whether assistance or prompting is required, and whether the activity was completed — with any notes that matter.
It also identifies your important routine anchors: sleep and waking, meals and hydration, movement and activity, appointments and responsibilities, and social connection and meaningful activity.
The functional pathway behind the tool
A person may understand what they need to do but still experience difficulty remembering, initiating, sequencing or consistently completing the routine in everyday life. The planner makes each of those steps visible — and shows where a cue or a little support makes the difference.
What the planner records
- The time or part of the day
- What you want or need to do
- The reminder or cue that may help
- Whether assistance or prompting is required
- Whether the activity was completed, and any relevant notes

An important distinction
“Supported performance is not automatically independent capacity.”
The planner is not a medical treatment tool, and a completed planner is not proof that somebody is independent. Where another person provides reminders, prompting or physical assistance, that support should remain visible in the ‘Help needed?’ column.
The purpose is to identify what the person can currently do, what support is required, what helps them initiate and complete the activity, and whether capacity becomes safer and more consistent over time.
