My Goals & What Matters to Me
A starting point for naming what matters to you, what you already do well, and your first three practical goals.
Every IMHC learning plan begins with the person. This tool helps you put into words what matters to you right now, what you would like to do more independently, and the people, places and activities that are important in your life.
It also asks what has become harder or less consistent — not to focus on problems, but so that goals are realistic and start from where you actually are.
Your first three practical goals become the anchor for everything that follows: the routines you plan, the skills you practise and the progress you review every 12 weeks.
