Prompt & Assistance / Prompt-Fading Record
Track the prompts and assistance actually used, whether a reduced prompt was tried, and the outcome — so support fades only when it is safe to.
Support should reduce as capacity grows — but only when performance is safe, reliable and consistent. This record makes prompt fading deliberate rather than accidental.
The Functional Assistance Scale runs from 'not yet attempted' through full assistance, substantial and some prompting, occasional prompting, to 'independent in this setting'.
For each session you record the starting prompt, the response, whether a reduced prompt was attempted, and the outcome and safety. Do not reduce support solely because a task was completed once.
