For treating professionals: recommendations, precautions and stop rules, priorities, adaptations — and what must remain with the qualified professional.
IMHC reinforces professional recommendations within scope; it does not replace them. This form lets a treating or qualified professional set out what is relevant to everyday implementation: current recommendations, precautions, contraindications and stop rules, functional priorities and suggested adaptations.
Critically, it separates what the IMHC and support team may reinforce from what must remain with the treating professional, and sets review timing and referral-back triggers.
