Baseline Functional Snapshot
Record actual performance before teaching or implementation begins — separating safety from assistance, and capturing the participant's voice.
A baseline is only useful if it records what actually happens. For each skill or task, this snapshot documents the current setting, initiation, sequencing and completion, the assistance provided, safety, and what the participant themselves says.
Safety and assistance are recorded separately: a task can be completed safely with substantial help, or completed independently but unsafely. Both matter.
A readiness level (from Engagement & Safety through to Sustained Self-Management) and a summary of priority barriers set the starting point for the learning plan.
